O.I. = (MAP X FiO2 X 100)/PaO2

For instance, if MAP is 10, FiO2 is .70 and PaO2 55, your O.I. is slightly >12. When the O.I. gets as high as 12 you must call the Medical Director. (Do this in anticipation of possibly achieving the study threshold level of 15.)
